What is an Anticoagulation Clinic?
The Anticoagulation Clinic, also known as Coumadin Clinic, is a simple, convenient way to monitor and assist patients taking warfarin. Our clinic is staffed by trained, clinical pharmacists to provide education and dosing adjustments of warfarin under the supervision of the referring provider.

Why an Anticoagulation Clinic?
Patients taking anticoagulants need to be aware of the potential for serious side effects that can occur if the medication is not properly monitored. Some of these side effects can be life-threatening. Research has shown that participating in an anticoagulation clinic may decrease serious complications from anticoagulation medications.

What are the benefits of participating?
● Decreased risk of potentially life-threatening complications
● Immediate PT/INR results (Prothrombin Time/International Normalized Ratio)
● One-on-one contact with a clinical pharmacist
● Better anticoagulant monitoring and management
● Increased access to education and support

How does it work?
Patients of the clinic are seen by a member of our staff for appointments. During your appointment, a small drop of blood will be collected from a finger prick to run the test. Your PT/INR results are available in less than one minute. The results are reviewed with you, your medication will be adjusted as needed, and easy-to-read, written instructions are provided. The results are also shared with your doctor. At each visit, education is also provided on complications, diet and drug interactions related to taking warfarin.

The Wyandot Memorial Hospital laboratory performs clinical tests for diagnostic and treatment purposes. Our state-of-the-art lab is staffed with highly trained medical technologists. We perform blood tests, urine testing, drug testing, STD testing, virus identification, and body fluid analysis, in addition to other services.

For your convenience, we perform lab testing with your healthcare provider’s order – whether they’re on the local medical staff or at another facility. So even if you do not see a doctor here, we can still do your lab work.

Most lab services are performed on an outpatient basis. We strive to analyze results as quickly as possible without sacrificing accuracy. Wyandot Memorial Hospital’s Patient Navigator provides one-on-one guidance and assistance to patients, families and caregivers and helps with many needs. The principal function of the Patient Navigator is to eliminate any barriers to timely screening, diagnosis, treatment and supportive care for each patient while also promoting smooth and timely continuity of care. The Patient Navigator works with a variety of patients – not just oncology patients or patients with chronic diseases – but rather any patient facing diagnostic testing or requiring assistance in any aspect of their medical care.

How can the Patient Navigator help me?

• Arrange transportation to and from appointments
• Identify resources for financial assistance including medication, co-pay assistance and other community resources
• Coordinate continuity of care for WMH patients who need to have surgery or treatment at another facility (i.e. The James Cancer Hospital, etc.)
• Assist with keeping track of doctor visits, tests and other appointments
• Help completing medical forms including those relating to insurance, Medicaid and Medicare
• Answer general questions about health insurance
• WMH Patient Navigator is a Certified Application Counselor for the Health Insurance Marketplace and can enroll patients and help them purchase health coverage through this program.

The physical therapy and occupational therapy programs at Wyandot Memorial Hospital are run by a team of professionals who build customized programs to fit the needs of each and every patient. We have three different programs for both therapies – inpatient, outpatient, and at-home services. Through strategic planning and dedicated effort, we help our patients regain their independence and enjoy a higher quality of life. We also provide one-on-one care for our patients, and this approach enables us to offer the quality care that every patient at our facility deserves.

Both physical therapy and occupational therapy have an incorporated Swing Bed Program (SWP). SWP is for patients who no longer need intensive acute therapy in the hospital but are not quite ready for home. This is an in-between stage between inpatient and outpatient that provides an opportunity for patients to continue recovering and training for a safe return home.

Physical Therapy

The goal of physical therapy is to help patients regain independence and mobility after an injury or illness renders them incapacitated. Physical therapy is more general than occupational therapy; in this program, we want to help patients improve all aspects of their lives, not just one particular area or skill. Physical therapists have many avenues in which they can achieve this.

We have a team of licensed physical therapists and licensed physical therapy assistants. Our team is experienced in working with all patients from pediatric to geriatric. Every patient works closely with a physical therapist and physical therapist assistants in order to build a recovery program tailored to their unique needs.

Occupational Therapy

In outpatient therapy, therapists learn more about each patient’s goals for recovery and build a more focused therapy program. For example, if a patient was a musician who suffered a hand injury, the therapy would have an emphasis on rebuilding hand strength and finger dexterity.

Our team includes three occupational therapists and three occupational therapy assistants. Like physical therapy, occupational therapy has inpatient, outpatient, home services programs and works with patients of all ages.

SPEECH LANGUAGE PATHOLOGISTS IN OHIO

Difficulty speaking is a frustrating problem for people of any age. Speech disorders are common in children and can be a major roadblock in their mental and social development. The speech language pathologists at Wyandot Memorial Hospital help patients of all ages overcome speech and swallowing disorders.

Speech language pathologists help patients with a variety of problems. In addition to speech disorders, they can also help people with other communication difficulties like dyslexia and auditory processing disorder. The goal of speech therapy is to help patients fully engage and interact with the world.

We often treat patients referred to us for:
• Expressive and receptive language disorders
• Difficulty with language fluency
• Difficulty with articulation

• Oral-motor dysfunction

• Social disorders

When to Look Into Speech Therapy
It can be difficult for a parent to determine when their child needs speech therapy. Every child develops differently, and it’s not always easy to figure out when they might need help. There are some markers for speech development in children that can point you in the right direction.

Your child may need speech therapy if:
• People have a difficult time understanding what they’re saying
• Your child takes a long time to say things
• Your child is silent for a long time before responding to simple questions
• Your child is over two and is having a hard time putting two words together
• Your child has difficulty making certain sounds like “th” or “ch”

UrgentCare is a walk-in clinic where no appointments are necessary. We provide same-day diagnosis and treatment for a variety of non-life-threatening injuries and illnesses. Most insurances are accepted, so be sure to bring your insurance card with you. Co-pays may be necessary.

What Does UrgentCare Treat?

UrgentCare is not emergency care. If you are experiencing a serious medical event, call 911 immediately. The purpose of UrgentCare is to help people with important, but non-life-threatening medical concerns as soon as possible.

Wyandot Memorial offers a multitude of health and wellness services through the Population Health and Wellness Department in our Wellness Center.

Some services include Cardiac and Pulmonary Rehabilitation, our diabetic self-management services, Living With Diabetes program, nutrition services, various support groups, the Matter of Balance program, and wellness gym membership services and SilverSneaker partnership.

Cardiac and Pulmonary Rehabilitation

Cardiac or Pulmonary Rehabilitation is a physician-ordered, medically-supervised program of exercise, emotional and social support with lifestyle modifications to help the heart and lungs regain strength. Rehabilitation reduces the risks of cardiovascular disease, improves pulmonary function and decreases mortality while providing a safe environment.

Our cardiac and pulmonary rehabilitation team include our Medical Director, a Registered Nurse, Registered Dietitian and an Exercise Physiologist. All staff members are certified in Advanced Cardiac Life Support and work together to determine blood pressure, interpret EKG monitoring and to further advance each patient. Continuous EKG monitoring is available based on your ordered diagnosis and insurance coverage. This program lasts approximately 12 weeks as advised by the ordering physician.

Diabetic Education

The mission of the Diabetic Team at this facility is to partner with the diabetic patient, physician and patient’s family to provide quality and comprehensive diabetes self-management education and tools. This will include empowering the patient to better manage his or her disease, improve their quality of life and prevent complications of diabetes.

The goals of the Diabetic Team are to provide knowledge and skill training, help individuals identify barriers to treatment, teach problem solving and coping skills to achieve self-management, and behavior change as needed. Diabetic Team members include the patient and family, primary care practitioner, diabetic teaching nurse, pharmacist, dietician, and exercise physiologist.

The diabetic teaching nurse works with each unique individual to identify his or her needs related to diabetes self-management. Education needs, behavioral goals and follow-up plans are established after a review of the physician’s health assessment, completion of a nurse assessment and review of a complete foot exam.

• Living with Diabetes offers diabetic self-management education training and support. Living with Diabetes is a 6-hour physician-ordered, multidisciplinary educational course with yearlong follow-up by the diabetic teaching nurse. After the full year, Medicare patients are eligible for two hours of continued nursing education per year if order by the physician.
• Individual education is offered for patients with special needs such as vision problems, hearing deficits, physical limitations, cognitive impairment, and language limitations.
• The Diabetic support group has roundtable discussions and guest speakers throughout the year.
• Other services offered include continuous glucose monitoring, glucometer instruction and insulin instruction.

Nutrition Services for Creating a Healthy Diet

Our Registered Dietitian can help you generate a healthy meal pattern to help you meet your personal goals.

• Outpatient Nutrition Services focuses on understanding your needs, routines, how to change unhealthy habits and gain healthy behaviors.
• Nutrition counseling and Medical Nutrition Therapy (MNT) diet education are offered for patients and families. This may include but are not limited to diabetes education, weight management, cardiovascular disease, renal disease, malnutrition, weight gain, cancer and chemotherapy, sports nutrition and/or maintaining a healthy diet.
• Portion Control and Holiday Recipes

Wellness Center Gym

The Wellness Center offers exercise programs for both healthy individuals and those with medical concerns including pre-diabetes, cancers, hypertension, arthritis or orthopedic concerns and comprehensive weight management.

Wellness Center membership includes an exercise consultation, wellness assessment, use of all equipment, blood pressure screening, full lipid panel bi-annually.

• General membership fees start at $30/month. Reduced pricing is available for multiple family members and volunteers of WMH.
• The Wellness Center participates in SilverSneakers and Renew Active to provide use of the gym at no out-of-pocket cost to those whose insurance plans include the benefit. Enrollment through SilverSneakers and Renew Active starts with your insurance carrier. Other Wellness Center services are available to participants for a small fee.
• Registration is required for Wellness Center memberships and classes.

TREATMENT FOR INFECTED & NON-HEALING WOUNDS

In many cases, wounds will heal on their own over time if properly cleaned and bandaged. However, some serious wounds have a difficult time healing and may even become infected. Additionally, people with diabetes and circulatory problems have a harder time healing wounds than other people. An open wound is nothing to take lightly. If you have a wound that has not shown any signs of healing after two weeks, you will want to have it treated by a professional as soon as possible.

Wyandot Memorial Hospital’s WoundCare provides advanced treatments for non-healing wounds. Our team can help you prevent infections, reduce pain, and explore treatments that will close the wound for good. We are staffed by a Certified Wound Nurse with in-depth knowledge of wound care and the most progressive dressings and wound vacs. We can also help with care, products and resources when caring for a new or established stoma.

STATE-OF-THE-ART DIAGNOSTIC TESTING

Imaging is used to obtain an inside look at the body’s bones, muscle, tissue, and organs. X-rays, which take pictures of the skeletal system, are an imaging service. Wyandot Memorial Hospital offers a variety of high-quality imaging services to assist in diagnosis and treatment. We have invested in state-of-the-art technology to create clear and detailed images so that we can provide patients with accurate information.

A specialist called a radiologist is responsible for analyzing imaging results. Our radiology team is highly experienced and familiar with all of the technology used at the hospital.
