You can access your medical records via the MyChart link at the top of this page, and you may also contact our Medical Records Clerk for assistance Monday-Friday from 7 am-3:30 pm by phoning 419-294-4991, extension 2211.
If you are picking up medical records, you may use this Release of Information PDF to print and complete prior to coming to the hospital. In addition to the Release of Information, your photo ID is also required to pick up records. Additional documentation may be needed for records of minors or circumstances involving a medical power of attorney.
