Home Health provides skilled nursing, rehabilitation and home health aide services on a short-term basis to homebound patients with a variety of medical conditions. These are delivered under the order of a medical provider. Medicare, Medicaid and private insurances often pay for these services.

Home Health Admission Criteria - for Medicare and most insurances

You must have a

  • Medical provider’s order for a skilled need (nursing or rehabilitation with physical, occupational or speech therapy)
  • Face-to-face visit with the medical provider ordering the home health services

You must be homebound

  • You have trouble leaving your home without help (like using a cane, wheelchair, walker or crutches; special transportation or help from another person) because of an illness or injury
  • Leaving your home is not recommended because of your condition and normally unable to leave your home because it is a major effort
  • You may leave home for medical treatment or short, infrequent absences for non-medical reasons.
