Anticoagulation Clinic

The Anticoagulation Clinic provides monitoring and dose adjustment for patients on warfarin (Coumadin). Upon referral from a physician, patients are seen by a pharmacist or nurse in the clinic. Using a point-of-care meter a small sample of blood is drawn from a finger prick. The meter provides the bleeding time results (PT/INR) in less than one minute. The pharmacist then provides written instructions to the patient for the new dose of warfarin. The results of the test are shared with the referring prescriber.
